A passport form query?

Hi!

I am filling the Indian Passport form after reading the guidelines given in the form.

There are so many columns which are not applicable to me, for example, change of name, any criminal proceedings etc.

what should I do with these columns? whether should I left it BLANK or fill with "Not Applicable" / "None" / "N.A."
