If you are U.S. born, the county of your birth should have record of it. They can certainly give you a copy. But they will spank your dumb azz for losing the other one.

Visit your local department of health services. I just obtained a copy of my childs birth record for school admission. There is a nominal fee for teh document but the process is very easy. You should be able to find information on your counties website.

If you were born in the US, you have to go to City Hall (where they keep the vital records) in the town where your birth was registered and request a copy.
You need to call the courthouse in the county that you were born in and request a copy. It usually cost a couple bucks, but they'll mail an official replacement out to you.
