Well, to qualify for the visa, basically you will need to prove 3 things. One, that the petitioner is in fact an American. Two, that the two of you have actually met. Three, that your foreign born fiancee doesn't intend to marry you simply for immigration benefits. The first 2 are relatively easy, or should be. So, how do you prove the 3rd. Photos of the two of you together are useful. Some people submit phone records showing that the two of you have remained in contact, some people like to submit letters. The fact is, there is no one way, or required way to do this.
Strictly speaking, there is no legal requirement for the American petitioner to be present at the interview, although you can certainly go if you like. At the very least, there will be no question that the two of you have actually met.
As for processing time, 8-12 months isn't that unusual. USCIS gets lots of these petitions, so 6 months or so to get the petition approved is frequent. Then the petition has to be sent to the State Department central office and from there to the embassy overseas. Your fiancee will have to do most of the things that any immigrant has to do, like provide police certificates and take an immigrant physical. Some embassies have shorter or longer backlogs, so, no one can be precise on how long it will take to actually get the visa.
